  2. Attach photocopies of all academic transcripts, testimonials, and certificates with the printed form.
  3. Two recent passport-sized photographs, , must be attached.
  4. Attach receipt of admission test/registration fee with this form.
  5. Incomplete forms will not be accepted under any circumstances.
  6. Admission is subject to verification of original documents at the time of enrollment.
  7. Follow the notice board/college website for admission test dates and merit list publication.
This page must be printed along with the admission form and submitted to the college office (College Form and prospectus should be collected before taking roll).
